Membership Application

Applications for Spring 2023 are now closed. Please check back Fall 2023.

 

Membership Requirements

​To be a member, interested individuals must complete the following:

  • Fill out the membership application

  • Pay membership fee ($13, shirt included)

  • Maintain active membership

  • Be HBV and HIPAA trained

    • All Team HBV members must attend the mandatory HBV training ​

    • HIPAA training is required for shadowing and health fairs to protect patient privacy

  • Attend 1 social OR 1 fundraiser

  • Participate in 1 Hepatitis B Awareness Week (HBAW) event (for Spring only)

The Point System

This point system will be used to determine active membership status:

​

  • Earn a sufficient amount of points in order to:

    • Stay an active member = 10 points every quarter

    • Shadow = 12 points every quarter

 

  • How to get points:

    • Socials = 1

    • Fundraisers = 1 per shift

    • Meetings = 1

    • Health Fairs = 1

    • Mentor-mentee hangouts = 1

    • Attend Team HBV Collegiate Conference = 1 point per day attended

    • End of quarter survey = 1 each

    • Photo submission = 0.5
